I, the undersigned do hereby apply for membership in the Fire Emerald Society

of Southeast Georgia. Further, I proclaim to be of good character and a law abiding

citizen of the United States of America. The Board of Directors may request from

the applicant verification concerning the type of membership.

Membership Dues: $30 Annually (One time initiation fee of
$10) Please make checks payable to the Firefighters Emerald
Society of Southeast Georgia or F.E.S.

All memberships will renew each January.
